On your iPhone, when it matters most
In an emergency, paramedics and ER staff check your phone's Medical ID from the lock screen — before they know anything else about you. That's where "who speaks for me" belongs, so it's the first thing they find, not the last.
Open Health → tap your photo → Medical ID → Edit.
Add the person who speaks for you as an emergency contact.
Paste your Care Wishes link into Medical Notes, and turn on Show When Locked.
This is the most private conversation you'll ever have — and it's built to stay that way. Your words travel inside the link, encrypted in the part of the address that never leaves your device. Nothing is stored on our servers. Private by design, because some things should be.
You've just been trusted with someone's voice.
They named what matters, so you'd never have to guess. When you're ready, put your own wishes into words — it's a conversation, not a form.
