Why this, and not another feature

Death is always in the room. What's been missing was never a cleverer tool — it was a place safe enough to finally speak.

You can say things to no one that you can't yet say to the people you love. That's not avoidance; it's how the words get found. So the point of CareGoals on your phone isn't a flashy AI trick to perform for. It's the opposite: presence, not performance. Your conversation is saved only on your device. You can rehearse the truth in private, and only then decide who gets to hear it.

Private on your device, and Apple-native where it counts

  • Saved only on your device. Your conversation lives in your own browser, not on our servers.
  • Apple Intelligence Writing Tools work right inside the conversation on a supported iPhone — on-device help to find or soften a word, if you want it. You never have to; the conversation is yours.
  • Add your wishes to Medical ID. Put your health-care proxy and a note pointing to your care wishes in the Health app's Medical ID — visible from the lock screen, where first responders look. Health app → Medical ID → Edit.
  • Install it as an app. In Safari, tap Share → Add to Home Screen. CareGoals opens full-screen, like any app.
  • Carry it. A printed wallet card and a QR that opens your document — from your emergency door card and your Care Wishes.
The honest caveat

The thinking isn't fully on-device — yet

Today, when Sage replies, your words are sent to AI in the cloud to compose a thoughtful answer, then let go — used to respond to you, and not kept or used to build a profile of you. That's private, but it isn't the same as never leaving your phone at all. We won't pretend it is.

Apple's on-device Foundation Models — the engine behind Apple Intelligence — let an app run a whole conversation on the device itself, with nothing sent anywhere. That's the version we're building toward: the most private conversation you'll ever have, held entirely in your own hand. When it's ready, it will change nothing about the words — only who could ever overhear them. No one.

The threshold is open. You don't need a reason, or permission from anyone else.
